WHAT A SPORTS MEDICINE DOCTOR ACTUALLY DOES, AND WHEN YOU NEED ONE

Sports medicine is one of the most misunderstood clinical specialities. Most people assume it is the preserve of professional athletes, the discipline that manages Premier League hamstrings and Olympic knee reconstructions. In practice, the majority of sports medicine consultations involve working professionals, active parents, and recreational athletes dealing with the same problems that affect anyone who moves their body regularly.

If you have been managing a niggling injury, had imaging that did not explain your pain, or been told to rest something that keeps coming back, a sports medicine assessment is often the missing step.

WHAT A SPORTS MEDICINE DOCTOR ASSESSES

A sports medicine physician is a fully qualified doctor who has undergone specialist training in the assessment, diagnosis, and non-surgical management of musculoskeletal conditions, injuries and disorders affecting muscles, tendons, joints, ligaments, and bone.
Unlike a general practitioner, a sports medicine doctor has the time, training, and tools to assess the musculoskeletal system in detail. A consultation typically covers:

  • A detailed history of the injury, including mechanism, progression, aggravating and relieving factors, and previous treatment.
  • A physical examination assessing range of motion, strength, neurological function, and specific orthopaedic tests relevant to the presenting problem.
  • In-clinic musculoskeletal ultrasound — the tool that most distinguishes sports medicine from other clinical approaches.
  • Where appropriate, an assessment of the contributing factors: biomechanical patterns, training load, equipment, and general health markers that affect tissue resilience.

The output is a diagnosis — not a differential, not a referral for further imaging, not ‘rest and see how it goes’. A working diagnosis, a management plan, and a clear understanding of the expected timeline.

MUSCULOSKELETAL ULTRASOUND — WHY IT CHANGES THE DIAGNOSIS

Musculoskeletal ultrasound is a real-time imaging tool used in the consultation room. Unlike MRI, which produces static images of anatomy, ultrasound allows the clinician to visualise structures as they move — tendons gliding, muscles contracting, fluid shifting — and to identify the specific source of pain with a precision that clinical examination alone cannot provide.
For common presentations such as shoulder impingement, rotator cuff tears, Achilles tendinopathy, plantar fasciitis, and knee pain, ultrasound in the consultation room allows the clinician to:

  • Identify the precise structure involved and quantify the extent of any pathology.
  • Correlate what is seen with what the patient feels — confirming or challenging the clinical diagnosis in real time.
  • Guide injections with precision, placing medication exactly at the target structure.
  • Avoid unnecessary MRI referral in the majority of cases, reducing time to treatment.

This matters in practice because a significant proportion of MRI findings are incidental — present in people with no pain, and not the source of pain in people who have it. A sports medicine doctor with MSK ultrasound skill is interpreting the imaging in clinical context, not simply reporting on what is structurally present.

CONDITIONS THAT RESPOND BEST TO SPORTS MEDICINE

Sports medicine is most valuable for conditions that are commonly managed sub-optimally elsewhere:

Tendon problems

Rotator cuff, Achilles, patellar, and gluteal tendinopathy are among the most common and most poorly managed musculoskeletal conditions in active adults. They do not respond to rest alone. They require progressive loading rehabilitation — and in many cases, adjunct treatments including shockwave therapy or ultrasound-guided injection to allow rehabilitation to begin. Sports medicine physicians understand this and manage accordingly.

Joint pain with inconclusive imaging

Knee, hip, and shoulder pain that has been investigated with X-ray or MRI and produced findings that do not clearly explain the symptoms is one of the most common reasons for sports medicine referral. A sports medicine assessment recalibrates the diagnosis around function and load rather than structural appearance.

Overuse injuries in runners and cyclists

Stress fractures, IT band syndrome, shin splints, and patellofemoral pain are load-related conditions that require sports medicine thinking — an understanding of training load, running biomechanics, bone stress physiology, and the tissue response to progressive overload. A standard GP or physiotherapy model is not always equipped for this complexity.

Return to sport after surgery or significant injury

Medical clearance for return to sport is a clinical decision, not a time-based one. A sports medicine physician can assess functional readiness — strength symmetry, movement quality, cardiovascular capacity — and make a recommendation grounded in objective criteria rather than the passage of time.

SPORTS MEDICINE AND PHYSIOTHERAPY, COMPLEMENTARY, NOT INTERCHANGEABLE

A common question is whether to see a sports medicine doctor or a physiotherapist. The honest answer is that they serve different functions and often both are appropriate.
A sports medicine physician provides the diagnosis — the clinical and imaging assessment that identifies what the problem is and what is causing it. A physiotherapist delivers the rehabilitation — the progressive loading program that resolves it. In the best clinical environments, the two work together from the outset.
Where patients run into difficulty is when they enter the rehabilitation pathway without a clear diagnosis, or when they have a diagnosis that is not precise enough to direct appropriate treatment. Starting rehabilitation based on a wrong or incomplete diagnosis is one of the primary reasons injuries become chronic.

WHEN TO BOOK A SPORTS MEDICINE APPOINTMENT

If any of the following apply, a sports medicine consultation is worth prioritising over waiting to see whether things improve:

  • You have had pain for more than six weeks that has not clearly improved with rest or physiotherapy.
  • You have had imaging — X-ray or MRI — and the findings do not match your symptoms or have not led to a clear management plan.
  • You are a runner, cyclist, CrossFit athlete, or team sport player with a recurring injury pattern.
  • You are considering a corticosteroid injection and want it placed accurately under ultrasound guidance.
  • You have been told to rest something that keeps coming back every time you return to activity.

At UPANDRUNNING, our sports medicine doctors use musculoskeletal ultrasound in every relevant consultation. No referral is required, and same-day appointments are available across our five Dubai locations.
